RESUMEN Introducción: El rinofima es una enfermedad de la cual se han descrito 4 clasificaciones clínicas asociadas a la gravedad de la afectación dérmica. Aun así, existen casos en quienes no son clasificables por ninguna de las alternativas. Objetivo: Mostrar un paciente no clasificable, según las clasificaciones preexistentes del rinofima. Caso clínico: Se presenta un paciente masculino de 68 años de edad, de piel blanca, con diagnóstico de rinofima, que en el examen físico se observa engrosamiento nasal, nodular, asimétrico, con piel normocoloreada, presencia de telangiectasias en toda su superficie y compromiso de más de una subunidad anatómica nasal. Conclusiones: El caso presentado no se puede enmarcar en ninguno de los ítems de las clasificaciones existentes, lo cual muestra la necesidad del diseño de una nueva clasificación del rinofima más exhaustiva e inclusiva para pacientes con este diagnóstico.


ABSTRACT Introduction: Rhinophyma is a disease of which four clinical classifications associated with the severity of dermal involvement have been described. Even so, there are cases in which they are not classifiable by any of the alternatives. Objective: To show an unclassifiable patient, according to the preexisting classifications of rhinophyma. Clinical case: A 68-year-old male patient is presented, with white skin, with a diagnosis of rhinophyma, who in the physical examination shows nasal, nodular, asymmetric thickening, with normal-colored skin, presence of telangiectasias on all its surface and compromise of more than one nasal anatomical subunit. Conclusions: The case presented cannot be framed in any of the items of the existing classifications, which shows the need to design a new, more exhaustive and inclusive rhinophyma classification for patients with this diagnosis.

ABSTRACT

Rinofima é uma doença desfigurante e progressiva do nariz, com alta prevalência. Apesar de ser considerada benigna, muitos pacientes têm procurado tratamentos curativos devido a deformidade estética e estigmatização social. Existem diversas técnicas cirúrgicas descritas para o tratamento desta doença. O objetivo deste relato é apresentar um caso que necessitou de abordagem terapêutica pouco utilizada para tratamento de rinofima devido às múltiplas comorbidades do paciente. O procedimento foi realizado sem complicações no intra e pós-operatório, e o paciente mantém seguimento ambulatorial com bom resultado estético final a longo prazo


Rhinophyma is a disfiguring and progressive disease of the nose with high prevalence. Despite being considered a benign condition, many patients have sought curative treatments due to aesthetic deformity and social stigmatization. There are several surgical techniques described in the treatment of this disease. This report aims to present a case that required an unusual therapeutic approach for the treatment of rhinophyma, given the patient's multiple comorbidities. The procedure was performed with no complications in the intra and postoperative period. The patient maintains an outpatient follow-up with a good long-term aesthetic result

ABSTRACT

Objective To observe the efficacy and safety of tacrolimus ointment combined with ornidazole and clarithromycin in the treatment of rosacea with facial hormone dependent dermatitis .Methods A total of 80 cases with rosacea complicated with facial hormone dependent dermatitis were selected , and they were randomly divided into observation group and control group according to the digital table ,40 cases in each group .The observation group was treated with 0.1% tacrolimus ointment besmear outside 2 times a day,a total of eight weeks,and at the same time,ornidazole and clarithromycin was taken orally for 4 weeks.The control group was treated with 0.1%tacrolimus ointment besmear outside 2 times a day for a total of eight weeks .The curative effects of two groups after treatment,and the symptom rebound rates aftre 4 weeks of treatment were compared .Results The total effective rate of the observation group was 92.5%,which of the control group was 67.5%,the difference between the two groups was statistically significant(χ2 =7.68,P<0.05).The symptom scores before treatment between the two groups had no statistically significant difference[(19.93 ±0.76)points vs.(19.89 ±4.07)points,P>0.05].After treatment, the symptom scors between the two groups had statistically significant difference (P<0.05).The bounce rate of the observation group was 7.5%,which of the control group was 31.58%.The difference between the two groups was statistically significant(χ2 =5.97,P<0.05).Conclusion 0.1%tacrolimus ointment combined with ornidazole and clarithromycin in the treatment of rosacea complicated with hormone-dependent facial dermatitis has satisfied curative effect and security ,the comprehensive treatment is better than single treatment .

INTRODUÇÃO: Rinofima é uma condição estigmatizante para o indivíduo portador. Descrita desde a época de Hipócrates, a doença, popularmente conhecida como nariz de alcoólatra ou elefantíase nasal, é responsável por gerar uma enorme deformidade no nariz. Oriunda de um processo de hiperplasia e hipertrofia das glândulas sebáceas da região cutânea nasal, associadas à fibrose e dilatação dos vasos sanguíneos e tecido conjuntivo locais. O tratamento cirúrgico é considerado o procedimento de escolha para melhoria estética. MÉTODOS: Estudo retrospectivo baseado na análise dos resultados de 11 pacientes, sendo dez homens e uma mulher, submetidos a tratamento do rinofima entre janeiro de 2010 e janeiro de 2016 no serviço de Cirurgia Plástica do Hospital Universitário da Universidade Federal de Juiz de Fora e Clínica Plastic Center. RESULTADOS: Dos 11 pacientes operados, um homem de 72 anos apresentou carcinoma basocelular invasor expansivo com margens cirúrgicas comprometidas após análise anatomopatológica da peça cirúrgica, obtendo resultado anatomopatológico de margens livres após reoperação. Outros três pacientes apresentaram hipercromia na região de intervenção durante o pós-operatório imediato, mas obtiveram resultados satisfatórios 6 meses após procedimento. CONCLUSÕES: Apesar dos avanços tecnológicos atuais, a retirada da lesão com bisturi frio e aplicação dos curativos pós-operatórios ainda demonstram ser altamente eficazes quando associadas, proporcionando excelentes resultados estéticos, com baixos índices de complicação e altos índices de satisfação dos pacientes, que em sua maioria obtiveram melhora da qualidade de vida após o procedimento.


INTRODUCTION: The rhinophyma is an extremely stigmatizing condition for a patient. The first descriptions of this disease dates back to the time of Hippocrates, which was popular known as alcoholic nose or nasal elephantiasis is responsible for generating a huge deformity in the nose. This deformity derives from a process of hyperplasia and hypertrophy of the sebaceous glands in nasal skin associated with fibrosis and dilated local blood vessels and connective tissue. Surgery is considered the treatment of choice for aesthetic improvement of the injury. METHODS: This was a retrospective study based on analysis of results from 11 patients (10 men and 1 woman) who underwent treatment for rhinophyma between January 2010 to January 2016 at the Plastic Surgery Service of the Teaching Hospital of Federal University of Juiz de Fora and Clinical Plastic Center. RESULTS: Of 11 patients who underwent surgery, a 72-year-old man had basal expansive invasive carcinoma with positive surgical margins after pathological examination of the surgical specimen, however, the patient had pathological free margins after reoperation. Other 3 patients had hypercromy in the intervention region during the immediate postoperative. CONCLUSION: Despite technological advances, the removal of the injury with a cold scalpel technique and the use of postoperative dressings are highly efficient, when combined, provide excellent aesthetic results with low complication rates and high rates of patients' satisfaction, whom mostly had improvement in quality of life after the procedure.

ABSTRACT

A rosácea pode manifestar-se com a formação de fima, que se caracteriza por hipertrofia de glândulas sebáceas e proliferação de tecido conectivo e vasos sanguíneos. Considerada complicação grave, a fima é mais comum em homens, e mais frequente no nariz, quando é denominada rinofima. Rosácea e rinofima podem trazer prejuízos funcionais e estéticos com piora da qualidade de vida dos pacientes. Nos estádios iniciais realizam-se terapêuticas farmacológicas, às quais, porém, a forma fimatosa responde pobremente. Relatamos um caso de correção cirúrgica de rinofima gigante pela técnica de shaving e eletrocoagulação, com excelente resultado cosmético e funcional.


Rosacea can develop into phyma, which is characterized by hypertrophy of sebaceous glands and proliferation of connective tissue and blood vessels. Regarded as a serious complication, phymas are more common in men, being more frequent in the nose ­ when it is called rhinophyma. Rosacea and rhinophyma can cause functional and aesthetic impairment, leading to a worsening in the patients' quality of life. Pharmacological and physical therapies are performed in initial stages, however phymatous variants of the condition respond poorly to the first. The authors of the present article report a case of surgical correction of a giant rhinophyma using the shaving and electrocoagulation techniques, with excellent aesthetic and functional outcomes.

Introdução: Rinofima é uma inflamação crônica dos tecidos do nariz, caracterizada por hipertrofia e hiperplasia progressivas das glândulas sebáceas e do tecido conjuntivo. Determina um aspecto de elefantíase nasal, secundária à congestão dos vasos da derme. Sua etiologia está associada, na maioria dos casos, ao uso abusivo de álcool. É considerada por alguns autores como sendo um estágio avançado de acne rosácea. O artigo tem como objetivo relatar um caso de rinofima, tratado cirurgicamente no Serviço de Cirurgia Plástica do Hospital Universitário da Universidade Federal de Santa Catarina com decorticação e eletrocoagulação. Método: Foi realizado revisão de prontuário e registro fotográfico de um caso de rinofima. Resultados: Paciente foi submetido a tratamento cirúrgico com evolução favorável. Conclusão: Existem diversos tratamentos para rinofima, sendo que a decorticação e a eletrocoagulação constituem uma excelente opção terapêutica.


Introduction: Rhinophyma is a condition involving chronic inflammation of the nose and is characterized by progressive hypertrophy and hyperplasia of sebaceous glands and connective tissue. Rhinophyma leads to an appearance of nasal elephantiasis, which is caused by the congestion of dermis vessels. Its etiology is mostly associated with alcohol abuse. Rhinophyma is considered by some researchers to be an advanced stage of acne rosacea. Here, we report a case of rhinophyma that was surgically treated with decortication and electrocoagulation at the Plastic Surgery Service of the University Hospital of the Federal University of Santa Catarina. Methods: A review of medical and photographic records of a case of rhinophyma was conducted. Results: The patient was underwent surgical treatment with favorable outcomes. Conclusion: There are several treatments for rhinophyma, with decortication and electrocoagulation being an excellent therapeutic option.

ABSTRACT

Objective To evaluate effects of artesunate on rosacea-like inflammation in mouse models.Methods Twenty-five male BALB/c mice aged 7 weeks were injected subcutaneously with 40 μ1 antibacterial peptide LL-37 into the back once every 12 hours for 4 sessions to establish mouse models with rosacea-like inflammation.These 25 mice were randomly and equally divided into 5 groups:after each injection of LL-37,model group were gavaged with sodium chloride physiological solution,treatment groups gavaged with 25,50 and 100 mg/kg artesunate solution separately,and positive control group gavaged with 30 mg/kg doxycycline hydrochloride solution.Another 5 healthy mice injected subcutaneously with pure water into the back for 4 sessions served as blank control group.Forty-eight hours after the initial injection of LL-37,changes in skin lesions and the intensity of erythema were assessed.Skin tissues at the dorsal injection site were resected and subjected to HE staining,the tissue structure was observed and the number of inflammatory cells was counted.Enzyme-linked immunosorbent assay (ELISA) was performed to estimate the activity of myeloperoxidase (MPO) in skin lesions.Results The model group showed obvious inflammatory reactions,and significantly increased erythema score (3.20 ± 0.84),inflammatory cell count (517.27 ± 99.43) and MPO activity (0.57 ± 0.08) compared with the blank control group (all P < 0.01).The positive control group showed significantly decreased erythema score (1.60 ± 0.89),inflammatory cell count (270.93 ± 124.63) and MPO activity (0.40 ± 0.05) compared with the model group (P < 0.05,0.01,0.01,respectively).Moreover,the erythema score,inflammatory cell counts and MPO activity were all significantly lower in 50-(1.80 ± 0.84,286.00 ± 33.72,0.43 ± 0.05,respectively) and 100-mg/kg artesunate groups (1.40 ± 0.55,258.00 ± 36.44,0.40 ± 0.06,respectively) than in the model group (P < 0.05 or 0.01).However,there were no significant differences in the erythema score,inflammatory cell count and MPO activity between 50-or 100-mg/kg artesunate group and the positive control group (P > 0.05).Conclusion Artesunate can inhibit rosacea-like inflammatory reactions in mouse models,especially the middle-and high-dose artesunate.

El rinofima es una enfermedad de la piel de la nariz, consistente en una hipertrofia de las glándulas sebáceas locales que conlleva a una deformación característica. Se presenta con hipertrofia de la piel, inflamación crónica, aumento del tejido vascular, con múltiples telangiectasias y comedones, todo lo cual ocasiona una punta de la nariz deformada. Afecta con más frecuencia al sexo masculino y puede llegar a malignizarse. Se presentan dos pacientes afectados por rinofima, que fueron atendidos en el Servicio de Cirugía Maxilofacial del Hospital Universitario Dr. Gustavo Aldereguía Lima de Cienfuegos. Se describe la técnica quirúrgica empleada en ambos casos. Se trata de una enfermedad rara, por lo que se considera de interés su estudio y divulgación.


Rhinophyma is a nose skin disease consisting on a hypertrophy of the local sebaceous glands which leads to a characteristic deformation. It presents with skin hypertrophy, chronic inflammation and increase of the vascular tissue, with multiple telangiectasias y comedones, all of which causes a deformed tip of the nose. It more often affects the male sex and can get to a malignancy. Two patients affected by rhinophyma are presented who were assisted at the maxillofacial service of the General Teaching Hospital Dr Gustavo Aldereguía Lima, Cienfuegos. The surgical technique used in both cases is described. It is a rare disease therefore it is considered of interest for study and dissemination.

ABSTRACT

O rinofima tende a determinar importantes alterações nasais, com prejuízos estéticos e funcionais, especialmente nos casos mais avançados. Apesar das diversas técnicas já descritas, a busca do tratamento ideal persiste. Dois pacientes com rinofima foram submetidos à excisão tangencial associada à eletrocoagulação, com posterior tratamento cosmético intenso. A técnica descrita se mostrou eficaz, segura, com excelente resultado estético e sem recidivas ao longo do período avaliado. Essa proposta de tratamento combinado configura-se como excelente alternativa para o tratamento do rinofima, devido a sua simplicidade técnica e seus bons resultados em longo prazo.


Rhinophyma tends to determine important nasal changes, with aesthetic and functional impairment, especially in more advanced cases. Despite the various techniques already described, the search for the ideal treatment persists. Two patients with rhinophyma underwent tangential excision associated with electrocoagulation, with subsequent intensive cosmetic treatment. The described technique is effective, safe, with excellent aesthetic results and no recurrences during the study period. This proposed combined treatment appears as a great alternative for the treatment of rhinophyma due to its technical simplicity and its good long-term results.

ABSTRACT

La rosácea es una dermatosis crónica de origen desconocido, principalmente localizada en la cara, caracterizada por episodios de: rubefacción, edema, enrojecimiento con aparición de pápulas, pústulas, telangiectasis y fibrosis. El rinofima representa la etapa avanzada de la rosácea, mostrando una hiperplasia e hipertrofia de las glándulas sebáceas a nivel nasal, con apariencia engrosada e irregular, lobulada de coloración rojo-púrpura con presencia de poros dilatados. Se han propuesto varios factores: genéticos, luz solar, calor, frío, esteroides, consumo de alcohol (no demostrado) presencia de Helicobacter pylori, infecciosos y seborreicos. También puede representar una reacción de hipersensibilidad al ácaro Demodex folliculorum. Para su tratamiento se utiliza: antibióticos, retinoides, crioterapia, radioterapia, escisión tangencial y escisión de espesor completo con cierre directo, con o sin injertos de piel. En los casos de rinofima con grandes tumoraciones y compromiso de las vías aéreas, la mejor elección de tratamiento es quirúrgico.


Rosacea is a chronic skin disease of unknown origin, mainly located on the face, characterized by episodes of flushing, edema, redness with papules, pustules, telangiectasia and fibrosis.The rhinophyma represents the advanced stage of rosacea, showing hyperplasia and hypertrophy of the sebaceous glands on the nose, with thickened, irregular, lobed appearance, red-purple color and dilated pores. Several factors have been proposed, like:genetic, sunlight, heat, cold, steroids, alcohol consumption (not shown), and the presence of Helicobacter pylori, infections and seborrhea factors. It may also represent a hypersensitivity reaction to Demodex folliculorum acarus. Antibiotics, retinoids, cryotherapy, radiotherapy, tangential excision and full-thickness excision with direct closure with or without skin grafts are used for it treatment. The best choice of treatment is surgical in rinophyma patients with big tumors that compromise the airways.

ABSTRACT

Rhinophyma is a progressively growing benign tumor that has the ability to distort the nasal form. Even though, it is considered a rare subtype of rosacea, it is of great importance for the ability to create: psychic, aesthetic and functional disorders. This report describes a severe case of rhinophyma in a 52 years old male patient, the medical treatment and the surgical technique used for removal.


El Rinofima es un tumor benigno de crecimiento progresivo con la capacidad de provocar severas alteraciones en la morfología nasal. Es considerado un subtipo de rosácea poco frecuente, sin embargo, es de gran relevancia por su capacidad de crear trastornos tanto estéticos como funcionales y psicológicos en las personas afectadas. Este reporte describe un caso severo de rinofima, en un paciente de género masculino de 52 años, el tratamiento médico y la técnica quirúrgica que se utilizó para su remoción.

Introdução: A rinofima é uma inflamação crônica dos tecidos do nariz com acometimento decor, textura e vascularização, apresentando crescimento exofítico irregular e telangiectasias.Objetivo: Levantar dados epidemiológicos de pacientes portadores de rinofima e avaliar osresultados dos diversos tratamentos para esta patologia realizados no Serviço de CirurgiaPlástica do Hospital Agamenon Magalhães (HAM), Recife, PE, Brasil, comparando-os comos tratamentos descritos na literatura. Método: Estudo retrospectivo, baseado na análise deprontuários de 28 pacientes submetidos a tratamento do rinofima, entre março de 2002 emarço de 2010. Resultados: A média de idade dos pacientes foi 50,1 anos, sendo 72,72%do sexo masculino e 17,28% do sexo feminino. O tratamento mais utilizado foi associaçãode decorticação/ dermoabrasão e eletrocoagulação, em 96% dos casos. Foram observadas asseguintes complicações: 1 caso de hipopigmentação, 2 de hiperpigmentação e 1 de cicatrizinestética. Em 3 casos, houve necessidade de mais de uma cirurgia para complementação dotratamento. Nenhum caso de câncer foi verificado nas peças cirúrgicas. O grau de satisfaçãodos pacientes foi de 100%. Conclusões: A rinofima é uma doença que possui tratamentosimples e eficaz, com excelentes resultados cosméticos.


Introduction: Rhinophyma is a chronic inflammation of the nose with involvement of color,texture and vascularization, with irregular exophytic growth and telangiectasia. Objective:To analyze epidemiological data of patients with rhinophyma and evaluate the results ofvarious treatments for this disease at the Service of Plastic Surgery of Hospital Agamenon Magalhães, Recife, PE, Brasil, comparing them with the treatments described in the literature.Methods: A retrospective study using analysis of medical records of 28 patients undergoing treatment of rhinophyma, between March 2002 and March 2010. Results: The average patient age was 50.1 years, with 72.72% male and 17.28% female. The most common treatment was an association of decortication/ dermabrasion and electrocoagulation,in 96% of cases. Complications observed: one case of hypopigmentation, two cases of hyperpigmentation and one case of unsightly scar. In three cases, there was need for more surgery to completion of treatment. No cancer was found on surgical specimens. The degree of patient satisfaction was 100%. Conclusion: Rhinophyma is a disease that has simple and effective treatment with excellent cosmetic results.

ABSTRACT

Introdução: O rinofima, também chamado de elefantíase nasal ou acne hiperplásica, temsido descrito desde o tempo de Hipócrates. Está associado, na maioria das vezes, ao hábitode ingestão de álcool (alcoolismo). É caracterizado histologicamente por hipertrofiae hiperplasia das glândulas sebáceas, proliferação fibrovascular da derme e acantose doepitélio. Método: Esse trabalho apresentar casos de rinofima tratados com equipamentode alta frequência (erroneamente denominado de radiofrequência, considerando suas característicasde alto grau de precisão de corte, assim como coagulação simultânea e possibilidadede isolamento oncológico da área tratada, pelas características de vaporizaçãocelular, ocorrida no local de ação do eletrodo). Seu mecanismo de ação é bastante similaràquele verificado no Laser de CO2. Resultados: Foram operados 8 pacientes portadoresde rinofima, com idades que variavam de 55 a 63 anos, dos quais 3 eram de sexo femininoe 5 eram do sexo masculino, entre os anos de 1990 a 2007. Os resultados foram naturais,com período de recuperação bastante curto. Nossa experiência há mais de 10 anos comeste tipo de equipamento levou-nos a somente utilizar esse método, em decorrência de suamaior facilidade e excelência dos resultados.


Introduction: Rhinophyma, also called nasal elephantiasis or acne hyperplasic, has beendescribed since Hipócrates. It is associated, in most times, to the continuous ingestion ofalcohol (alcoholism). histologicaly is characterized by hypertrophy and hyperplasia of thegreasy glands, fibrovascular proliferation of the dermis and acantosis of the epithelium.Method: This article presents cases of rinophy treated with equipment of high-frequency(also denominated of radio-frequency, due to their characteristics of high degree of cutprecision, as well as the simultaneous coagulation and possibility of oncologic isolationof the treated area, because of cellular vaporization, in the site of action of the electrode).His action mechanism is similar to that verified in the CO2 laser. Results: Eight patientspresenting Rhinophyma were operated, with ages that varied from 55 to 63 years, of which3 were female and 5 were male, among the years from 1990 to 2007. The results werequite natural, with quite short recovery period. Conclusion: Our experience has more than10 years with this equipment type only took us the to use this method, due to his largesteasiness and excellence of the results.

ABSTRACT

A rhinophyma lesion can mask the existence of coexisting occult skin cancers, and many types of tumors can mimic a rhinophyma. Although several coexistent malignant tumors have been reported in patients with rhinophyma, few reports have described the coexistence of rhinophyma and cutaneous squamous cell carcinoma (SCC). Herein, we report on a case of SCC in a 45-year-old man who presented with 2-month-history of a painful, erythematous, globular plaque on the tip of his nose and this lesion mimicked a rhinophyma lesion. The histopathologic examination showed a proliferation of atypical squamous cells that extended into the dermis, which was consistent with SCC. He was treated with surgical excision, and there has been no evidence of recurrence during the follow-up period of 1 year.

ABSTRACT

Rhinophyma is characterized by a disfiguring soft tissue hypertrophy of the nose. Severe cosmetic deformity and impairment of breathing may coexist, making the surgical treatment necessary. We present a case of giant rhinophyma, who had difficulty in nasal breathing and took preventive aspirin for CVA. We treated this patient with scalpel tangential excision and skin graft. A giant rhinophyma involved two thirds of nasal dorsum, nasal tip, and both alae. The patient withdrew aspirin 7 days prior to surgery, and rhinophyma was excised with scalpel. The defect was covered with full-thickness skin graft from right supraclavicular area. Five days after surgery, the patient resumed to take aspirin. A skin graft was taken completely and a pathologist confirmed an excised specimen to be a rhinophyma. Within 1 month postoperatively, the patient showed more natural nasal contour and improvement of nasal respiration. Six months after the operation, the nose represented a good appearance without recurrence of rhinophyma. We present a case of giant rhinophyma which had bleeding tendency because of preventive aspirin. We treated this case with scalpel excision and full-thickness skin graft, instead of secondary intention wound healing.

ABSTRACT

Rhinophyma is a disfiguring soft tissue hypertrophy of the nose. It is an uncommon disease that primarily affects Caucasian men in the fifth to seventh decades of life. Many treatment methods have been advocated, often with acceptable success, but there appears to be no one modality that is universally endorsed. We present a case of a 70 year old male who was treated with dermabrasion for rhinophyma in the nose.

ABSTRACT

Kimura's disease is a benign, uncommon, chronic inflammatory condition that usually presents with painless subcutaneous nodules or plaques. Head and neck are the most frequently involved sites in Kimura's disease. Mandible is the most commonly involved, followed by neck, cheek, scalp and forehead. Other possible sites are oral cavity, inguinal area and extremities, but there have been no reports involving the nose, especially the one that looks like a rhinophyma. We describe a case of Kimura's disease presenting like a rhinophyma.

ABSTRACT

Two patients with rhinophyma were treated using the laser CO2. The use of this technique showed many advantages. No bleeding occurred during the operative procedure despite the high degree of vascularity of the skin and soft tissue. The wound healed rapidly in 2 week. Minimal discomfortable such as paint, edema occurred in the postoperative period. The restoration of normal function and excellent cosmetics was possible without complication.
